For Tamariki under 2 years old, each age band includes all tamariki up to the following age band e.g. a tamariki who is 5 ½ months old would be included in the 5-12 mths age band, not 3-5 mths age band. Age bands above 2 years old are distinct. Data includes all Tamariki age between 0 - 18 years.</li><li data-block-key=\"dhtn9\"><i>Total population</i> - all tamariki regardless of vaccination status or eligibility. The total population is sourced from the consumer table within AIR. The consumer table contains a historical migration of individuals sourced from the NIR, CIR, HSU, and NES. The consumer table is kept up to date daily with a feed from AIR and PMS. The table is also updated daily with births and deaths, with a manual update if an individual is identified as overseas. The total population is filtered to individuals who are not deceased as at the report run date or flagged as overseas.</li><li data-block-key=\"7ddah\"><i>Eligible population</i> - tamariki are eligible for their first MMR dose at 12 months of age and their second dose at 15 months.</li><li data-block-key=\"d0nss\"><i>Population due for vaccination</i> - eligible population due to have an MMR vaccine. This is calculated as tamariki in the total population who have reached the age of eligibility for at least one dose and have outstanding vaccination activity as recorded in AIR. Tamariki may also be eligible if it has been over 4 weeks since their previous immunisation and they have outstanding vaccination activity as recorded in AIR.</li></ol><p data-block-key=\"59qk5\">AIR: The Aotearoa Immunisation Register is a computerised information system that has been developed to hold immunisation details of all New Zealanders. AIR fully replaced NIR and CIR in Dec-2023.</p><p data-block-key=\"82e9m\">NIR: The National Immunisation Register is a computerised information system that has been developed to hold immunisation details of New Zealand children. The NIR was introduced in 2005 and decommissioned Dec-2023.</p><p data-block-key=\"b8100\">CIR: The Covid Immunisation Register is a computerised information system that has been developed to hold Covid and Flu immunisation details of all New Zealanders. The CIR was decommissioned Dec-2023.</p><p data-block-key=\"3oj06\">HSU: The Health Service User table provides information about the number of people in New Zealand who used health services in the 2022 calendar year (01Jan22 - 31Dec22).</p><p data-block-key=\"1rg8f\">NES: The National Enrolment Service provides national enrolment and identity data.</p><p data-block-key=\"f5ot1\">PMS: Practice Management System</p><p data-block-key=\"a3rkg\">From Immunisation Handbook: The first dose of MMR is scheduled and recommended at age 12 months, if infants are given MMR from age 11 months in other circumstances, they are considered to have been given MMR1 and therefore only need one further dose, ideally given at age 15 months. To optimise early protection, dose two can be given from age 12 months, at least four weeks later. For toddlers and children older than 15 months, give both doses of MMR four weeks apart.</p><p data-block-key=\"5fp8u\">Immunisation Events are recorded in AIR. Historic data has been populated from AIR, NIR, and CIR. Current data is populated from the AIR vaccinator portal and PMS.</p><p data-block-key=\"cq85o\"><b>Data Sources</b></p><p data-block-key=\"flpps\">Data is supplied by Te Whatu Ora, updated weekly on Te Whata, and is sourced from:</p><ol><li data-block-key=\"2pq9o\"><i>National Enrolment Service (NES) -</i> A real-time view of patient identity and enrolment information maintained by practice staff via web-based services.</li><li data-block-key=\"b6q06\"><i>Aotearoa Immunisation Register (AIR)</i> Aotearoa Immunisation Register (AIR) is a computerised information system that has been developed to hold immunisation details of all New Zealanders. AIR fully replaced NIR and CIR in Dec-2023. The concept of opting off from the NIR has been discontinued and has been replaced by the concept of restricting access in AIR. An individual’s immunisation status will be obfuscated, and they will not be counted towards the total population. <a TO ACCESS HAUORA HUB VACCINE DATA</b></p><p data-block-key=\"a9trk\">These Terms and Conditions (Terms) set out the terms and conditions on which Te Kāhui Raraunga Charitable Trust (we, our, or us) agrees to permit you to access the Hauora Hub and any Vaccine Data within the Hauora Hub (the Hauora Hub), any related websites or applications operated by us, and any social media accounts related to the Hauora Hub.</p><p data-block-key=\"dpd25\">By accessing the Hauora Hub and the Vaccine Data within the Hauora Hub, you and/or the entity you represent (you, your) agree to be bound by these Terms, as altered by us from time to time. If you do not wish to agree to these Terms, including any updated version of these Terms, you must cancel your access to the Hauora Hub and the Vaccine Data within the Hauora Hub.</p><p data-block-key=\"4tbp0\"><b>INTRODUCTION</b></p><p data-block-key=\"7dkjf\">We are an independent body established in 2019 to lead the action required to realise the advocacy of the Data Iwi Leaders Group (Data ILG). We aim to enhance the social, cultural, environment, and economic well-being of Māori; to enable iwi, hapū, and whānau Māori to access, collect and use Māori data to measure and identify areas of Māori well-being requiring change; to support innovation and research that advances the use of Māori data; to educate Māori on the benefits of collection and use of Māori data; and to promote and advance opportunities for iwi, hapū, and whānau Māori to share knowledge and information relating to collection and use of Māori data.</p><p data-block-key=\"b5l6h\">Te Whata is a data platform tailored specifically by iwi for iwi – by us for us. Te Whata is for iwi members, technicians and leaders – designers and user of data. We want to be able to tell our own narratives according to our own priorities and world views. Te Whata has been designed with this in mind.</p><p data-block-key=\"62n9r\"><br/><b>1. DEFINITIONS</b><br/></p><p data-block-key=\"5q1t1\"><b>Agreed Purpose</b> means the purpose for which Data is collected, stored, and made available being:</p><p data-block-key=\"3rd3e\">a) to support planning, monitoring, invitation, delivery, and quality improvement of services related to the provision of the MMR, DTaP, Flu, and COVID-19 vaccines;</p><p data-block-key=\"5djt8\">b) includes the AIR Agreed Purpose; and</p><p data-block-key=\"4d0ka\">c) to enable iwi, hapū, and whānau to self-determine their own governance of data including its use to actively protect communities by ensuring equal opportunity, access, and support for immunisations.</p><p data-block-key=\"es28l\"><b>AIR Agreed Purpose</b> means the purpose for which Data is collected, stored, and made available from the Aotearoa Immunisation Register (AIR) being:</p><p data-block-key=\"gv2p\">a) managing individuals’ health;</p><p data-block-key=\"bsipo\">b) keeping the individual and others safe;</p><p data-block-key=\"3lmlp\">c) planning and funding health services;</p><p data-block-key=\"6g43b\">d) carrying out authorised research;</p><p data-block-key=\"33ohi\">e) training health care professionals;</p><p data-block-key=\"6a2f8\">f) preparing and publishing statistics;</p><p data-block-key=\"aopjd\">g) improving government services; and</p><p data-block-key=\"48bn9\">h) enabling broader health and social support services.</p><p data-block-key=\"5r2gf\"><b>Vaccine Data</b> means the MMR, DTaP, Flu, and COVID-19 vaccine data made available to Iwi Administrators on the Te Whata platform.</p><p data-block-key=\"elpjh\"><b>Iwi Administrator</b> or <b>You</b> means all persons who are authorised by Us to access Te Whata as an Iwi Administrator</p><p data-block-key=\"6ulgf\"><b>We, Us,</b> or<b> Our</b> means Te Kāhui Raraunga Charitable Trust.</p><p data-block-key=\"65mh9\"><b><br/>2. ADMINISTRATOR OBLIGATIONS</b></p><p data-block-key=\"39bhi\">The terms in this section apply to individuals, whānau, hapū, iwi, Māori entities and collectives, and other business entities, who access the Hauora Hub and any Vaccine Data within the Hauora Hub.</p><p data-block-key=\"88bqj\"><b>Registering as an Administrator</b></p><ol><li data-block-key=\"46rmf\">You may register with the Hauora Hub, and receive access to the Vaccine Data if you identify as the nominated represented on behalf of your Iwi.</li><li data-block-key=\"1do8f\">If you register with us on behalf of a whānau, hapū, iwi, Māori entity and collective, or other business entity, then you represent that you have authority to bind that whānau, hapū, iwi, Māori entity and collective, or other business entity to these Terms.</li><li data-block-key=\"3dq5c\">We may decline to register you for any reason, and we are not obliged to state such reason.</li><li data-block-key=\"cv1cb\">We may provide services and other opportunities to Iwi Administrators from time to time. It is in our sole discretion what services or opportunities we provide and who we offer those services and opportunities to. The terms of any services or opportunities will be communicated to you at the time these are offered to you.</li></ol><p data-block-key=\"60rrj\"><b>Profile</b></p><ol><li data-block-key=\"1gm0h\">When you register with us as an Administrator, you will submit to us information and other content related to you or the entity you represent.</li><li data-block-key=\"3aucq\">You agree you are responsible for keeping your information connected with you up to date.</li><li data-block-key=\"6e06e\">You agree that when you register for the Hauora Hub, and when you provide us with any information or materials through the Hauora Hub, or in any other way:<br/><br/>a) all information that you provide to us will be accurate, complete, and up to date, and you will provide us with all information requested;<br/><br/>b) you have all the rights necessary to provide the information and materials to us;<br/><br/>c) for any personal information you provide to us about any person, you have obtained that person’s consent to provide us with that information and for us to use that information in accordance with these Terms and in accordance with our Privacy Policy.</li></ol><p data-block-key=\"cjqpp\"><b>Ending your registration</b></p><ol><li data-block-key=\"2ceh9\">You can cancel your registration with the Hauora Hub at any time by giving us notice in writing.</li><li data-block-key=\"9tf8u\">If you end your registration with Hauora Hub, your Profile will be removed from the Hauora Hub, and you will not have access to the Vaccine Data.</li><li data-block-key=\"1ic8t\">We may terminate or suspend your registration with the Hauora Hub and the Vaccine Data at any time, if we (in our sole discretion) consider you have breached these Terms, breached any applicable law, or have any other concerns about your conduct, whether related to the Hauora Hub, the Vaccine Data, or not. You agree you will have no claim against us for any deactivation or removal in accordance with this clause.</li><li data-block-key=\"a1r3k\">Upon termination or suspension, you must delete or destroy any Vaccine Data held by you in any format (electronic or otherwise).</li></ol><p data-block-key=\"94mb\"><b>Access to Vaccine Data</b></p><ol><li data-block-key=\"9g87s\">You acknowledge the Vaccine Data is Confidential Information and will be held in the strictest of confidence and must not be disclosed to any third parties unless in accordance with these Terms.<br/></li><li data-block-key=\"adi2m\">You must:</li></ol><p data-block-key=\"fhhml\">a) only access and/or use the Vaccine Data for the Agreed Purpose; and</p><p data-block-key=\"bbig9\">b) not allow any third party to access and/or use the Vaccine Data other than for the Agreed Purpose.</p><ol><li data-block-key=\"5d2i2\">You agree the Vaccine Data must be accessed, stored, and used in accordance with all legislative requirements including the Privacy Act 2020 and such other requirements as are set out in the Health Information Privacy Code 2020 and HISO 10064:2019 Health Governance Guidelines (including any updates).</li><li data-block-key=\"aa5dq\">Refreshed data supplied under this agreement is to be retained until the subsequent data refresh is supplied.</li><li data-block-key=\"5t48p\">You are only authorised to access and/or retain any Vaccine Data until 30 June 2024. After that date, any Vaccine Data held by you must be securely deleted or destroyed.</li><li data-block-key=\"6uoj5\">You must comply with any reasonable directions we provide in relation to access to and, use of, the Vaccine Data.</li><li data-block-key=\"63dgj\">You must take all steps necessary to ensure the Vaccine Data is only accessed and/or used by persons who have a role that requires access to the Vaccine Data for the performance of duties aligned with the Agreed Purpose.</li><li data-block-key=\"8suaq\">You must immediately notify us if you become aware of any unauthorised access to the Vaccine Data or have any concerns about the security or integrity of the Vaccine Data.</li></ol><p data-block-key=\"519h5\">20. Where use of the Vaccine Data results in a personal invitation to an individual to receive a vaccination, you must ensure communications are developed to inform patients and consumers on how the information necessary to provide an invitation was obtained.</p><p data-block-key=\"6r59d\">21. Information in the AIR may be shared with iwi and other Māori providers in line with the Ministry’s obligations to honour Te Tiriti or Waitangi through the provision of data sharing. The purpose of this data share is to enable iwi, hapū, and whānau to self-determine their own governance of data including its use to actively protect communities by ensuring equal opportunity, access, and support for immunisations.<br/><br/> This data sharing may be at an aggregated or individual level and may also be used for statistical analysis and research purposes, in line with usual Te Whatu Ora processes in accordance with the privacy requirements set out in the Privacy Act 2020 and Health Information Privacy Code 2020.</p><p data-block-key=\"9551j\">22. You must not provide your Administrator log in details to any other party and must notify us if you consider that another person requires access to the Vaccine Data. Only authorised Administrators are authorised to access Vaccine Data available via an Administrator access page on Te Whata.</p><p data-block-key=\"augoo\">23. We source the Vaccine Data from government and are required to comply with terms and conditions regarding access to the Vaccine Data.</p><p data-block-key=\"a8lc5\">24. We have the right at any time to restrict and/or stop your access to the Hauora Hub and/or the Vaccine Data if this is necessary to enable us to comply with the Terms on which the Vaccine Data was provided to us.</p><p data-block-key=\"e6c5d\"><b>Errors and complaints</b></p><p data-block-key=\"fgus1\">24. You must contact us immediately if there is an error or defect in the Hauora Hub or Vaccine Data.</p><p data-block-key=\"d5or3\">‍25. We will use all reasonable endeavours to correct any such error, but, to the maximum extent permitted by law, will not be liable for any losses (whether direct, indirect, or consequential) arising from an error or defect within the Hauora Hub or the Vaccine Data.</p><p data-block-key=\"38ovm\">26. If you have any concern about the information that is available through the Hauora Hub, whether that information relates to you or someone else, you may let us know by contacting us at <a href=\"mailto:[email protected]\">[email protected]</a>. We will follow our internal processes in response to any concerns reported to us.</p><p data-block-key=\"kov3\">‍<b>Compliance with policies</b></p><p data-block-key=\"eokob\">27. By registering with the Network or using the Services or the Website, you agree to comply with any policies or other requirements issued by us from time to time, and those policies and other requirements will form part of these Terms.</p><p data-block-key=\"gil9\"><b>Privacy policy</b></p><p data-block-key=\"7fvrb\">28. Your privacy is important to us. We gather, process, use, store and disclose information about you in accordance with our privacy policy, which may be found <a href=\"/privacy-policy/\">here</a>. Our privacy policy forms part of these Terms, and by registering with the Hauroa Hub and accessing the Vaccine Data, you agree to our privacy policy.‍<br/><b>Hauora Hub integrity</b></p><p data-block-key=\"3q2sm\">29. You must not attempt to damage, interfere with or harm the Hauora Hub, or any network or system underlying or connected to the Hauora Hub in any way whatsoever.</p><p data-block-key=\"ct6ef\"><b>Amendments</b></p><p data-block-key=\"ej09o\">30. We may amend these Terms from time to time. Notice of any such amendments will be announced via the Hauora Hub and will be effective immediately, unless we state otherwise. You are responsible for reviewing these Terms regularly to obtain timely notice of any such changes.</p><p data-block-key=\"75lne\">31. Your continued registration as an Administrator and access to the Vaccine Data will constitute your acceptance of any changes or revisions to these Terms.</p><p data-block-key=\"cpr2u\"><b>No Warranties</b></p><p data-block-key=\"4c7j1\">31. You understand and agree that your registration as an Administrator, use and access to the Vaccine Data is at your sole risk.</p><p data-block-key=\"d06tg\">32. The Hauora Hub and access to the Vaccine Data are provided on an “as is” and “as available” basis. To the maximum extent provided by law, we disclaim and exclude any implied conditions or warranties (including, without limitation, any warranties of accuracy, completeness, reliability, merchantability and fitness for a particular purpose).</p><p data-block-key=\"e51rk\">33. Without limiting the above, to the extent permitted by law, we do not warrant that the Hauora Hub, or access to the Vaccine Data, will be uninterrupted, timely, secure, error-free or that any information or materials provided through the Hauora Hub, to the Vaccine Data are accurate, complete, or up-to-date.</p><p data-block-key=\"ac8ct\">34. All statements, information and recommendations made by us through the Hauora Hub are believed to be reliable, but do not constitute a guarantee or warranty.</p><p data-block-key=\"5bfro\"><b>Maximum Liability</b></p><p data-block-key=\"fjolk\">35. If we are found to have any liability under or in connection with these Terms, to the extent permitted by law, the maximum amount of our liability arising out of all claims under these Terms or relating to the Hauora Hub will not in any circumstances exceed NZ$100.</p><p data-block-key=\"9up4u\"><b>Waiver</b></p><p data-block-key=\"85vh3\">36. Our failure or delay to exercise any right or remedy we may have under these Terms will not be construed or operate as a waiver thereof, nor will any single or partial exercise of any right or remedy preclude the further exercise of such right or remedy.</p><p data-block-key=\"a6v4p\"><b>Governing Law</b></p><p data-block-key=\"7tgtu\">‍37. These Terms will be governed by the law of Aotearoa New Zealand.
